WHO WE ARE AND WHAT WE DO
With your support, we’re able to offer
CHCFC is a federally qualified health center, which means we are a local,
nonprofit medical office that serves areas and populations that don’t have
dical and dental services regardless o
enough access to healthcare. We provide primary-care services regardless
of one’s ability to pay. We meet very high standards for quality and safety,
insurance or ability to pay.
and we are governed by a board of directors, the majority of whom must be
health-center patients.
We offer primary-care, behavioral-health, and dental- care services.
We partner with our patients to help them live healthier lives, and we are dedicated to making healthcare easier to access. This means providing special services such as addiction treatment or gender-affirming care within the primary-care setting and not at a separate appointment or a different office. It also means making sure patients receive care in the language that is most comfortable for them, and by a provider who sees and respects their dignity as a human being.
In addition to high-quality care, we offer support services such as assistance with insurance, transportation, and help navigating the complex network of services available within our community.
We take nearly all insurance plans, and we have appointments with compassionate and attentive providers available.
